MV100 blowing output tubes

I have a friend with a MV100 who came to me last year, saying he couldn't set the bias on this amp. There were two bad output tubes and one weak pre driver. We made sure every tube tested like new, set the bias and he went on his way. The set didn't last the year before the left channel began blowing tubes. He tried several sets before he had to concede there was an underlying problem.
Before I dig into this too deeply I would ask if any of you have had a similar problem with their MV100?
Does anyone have the factory service manual. I have some hand drawn schematics, but I hate to trust that someone faithfully copied the design correctly.
Any help would be appreciated.

Hi. I know that this is an older post, but I am new to the forum, and happen to have a factor service manual, as well as an mv100 that is a eat's EL 34s for breakfast!! If you are still looking for the service manual, I will scan and send it to you.
